Monday- Friday: 8:00am-5:00pm 

$35 Physicals (School and Sport) 

Note: The School Code of Illinois requires that upon the child's first entry into public schools (Kindergarten or First grade), entry into 6th and 9th grades, and first-time entrance into an Illinois school, a student must present proof of having a physical exam by a licensed physician.
